The two main things that will effect the fuel mileage is the thermostat and O2 sensors. Disabling the EGR will drop mileage up to 2 MPG. Torque reduction may be playing a part in this as well. If the motors not limited to only make X amount of torque in overdrive and it now can make as much as the timing / fuel will let it, it may be using more gas. If all else fails put the stock tune back in it and see if the mileage comes back. I don't think the trans shifting and CAT code would effect the mileage at all, The trans shifting harder should even help it go up.
